Waters Pediatric Center

The National Committee for Quality Assurance (NCQA) states that a Patient Centered Medical Home is model of care that put patients at the forefront of care. Waters Pediatric Center has been recognized by the NCQA as a level 2 Patient Centered Medical Home.  This recognition shows that we are equipped with the tools, resources and systems to provide high quality patient-centered care.

Accessibility:

Same day appointments for routine and urgent care
Clinical advice from physician by telephone outside of regular office hours
Timely clinical advise through the interactive electronic system
Access to a web portal which allows timely access to health records, communication with the practice, prescription refills, referrals and access to test results

Team-Based Care:


New patients are provided with a new patient handbook and are assisted with the process of becoming oriented with the practice
Assistance is provided in the transitioning process between pediatric care and adult care.
We obtain and document a detail medical history  and information about all care obtained outside the practice
We provide equal access to all patients regardless of the source of payment and provide uninsured patients with information on obtaining insurance
We have a multilingual pediatrician and staff
We provide multilingual educational material

Focused attention is given to your child’s specific physical health, mental health, and acute and chronic health care needs
Regular preventative and wellness related care

Care Coordination: 

We follow-up on normal and abnormal lab values and imaging
We notify patients or families on normal and abnormal lab values and imaging
